With funding from the Centers for Disease Control and Prevention, NORC, in partnership with the Center of Health Care Strategies, will support the CDC Office of the Associate Director of Policy and Strategy (OADPS) in continuing and accelerating dissemination of information and practical tools for adoption of the 6|18 framework.
CDC’s 6|18 Initiative connects healthcare purchasers, payers, and providers with CDC researchers, economists, and policy analysts to find ways to improve health and control costs with effective interventions targeting six high-burden health conditions using 18 evidence-based prevention and control interventions. This initiative works with State Public Health and Medicaid teams to support implementation of their selected 6|18 interventions through technical assistance and resources via an online community known as "the Collab". NORC and CHCS will develop resources for cross-sector engagement based on existing information stored on the Collab, conducting interviews of state and public health officials and maintaining on-line discussion resources for the state teams using the Collab.
NORC and CHCS will be developing state spotlights of 6|18 Initiative innovations, updating existing technical assistance resources for use by states and public health officials, developing peer-to-peer engagement opportunities for states, interviewing state and public health officials and promoting the materials on the site.
The goal of the project is to accelerate cross-sector engagement across state public health and health care agencies and drive uptick in 6|18 Initiatives leading to better population health, including increases in: tobacco cessation, asthma control, controlling hypertension, and improving inappropriate antibiotic use practices.
