Supporting Incarcerated Individuals Transitioning to the Community: State and Health Plan Approaches and Lessons

​For Molina Healthcare Inc., NORC profiled and studied pre-release care coordination programs in three states - Maryland, New Mexico, and Ohio. These programs support soon-to-be released incarcerated individuals transitioning to the community.

NORC conducted a literature review of publically available information on pre-release programs as well as focused interviews with leaders from Medicaid programs and managed care organizations, as well as national policy experts and correction center staff. The purpose of these interviews was to gather further information that currently might not be publicly available and the thoughts and knowledge on pre-release programs and incarcerated individuals' healthcare and social needs from key national thought leaders and stakeholders.
