Community Health Worker Evidence Based Models

A project of the Rural Health Outreach Tracking and Evaluation Program

​The goal of this project is to identify evidence-based models (EBMs) that may benefit the 330A Outreach Authority Program grantees, document the scope of their use in the field, and build topic-specific EBM Toolkits to be housed within an EBM Warehouse on the RAC Website. NORC has completed a Community Health Worker Evidence Based Model Toolkit, Community Health Worker Report, and a promising practices issue brief detailing models for rural communities that are interested in implementing community health worker programs.

This project was conducted as part of the Rural Health Outreach Tracking and Evaluation Program.
